Health goals you select, eligibility answers, state checks, order history, and device data that could show an interest in a health service.

To check eligibility, route your information to a licensed provider, process orders and shipping, give support, keep the site secure and meet legal duties.

LocumTele and its licensed providers, the dispensing pharmacy, and service providers for payment, hosting, email and messaging, under contract and only for those purposes.

We do not sell your consumer health data or use it for targeted advertising.

Ask what we hold and who we shared it with, ask us to delete it, or withdraw consent. Email support@azurawell.com with the subject "Consumer health data request". We reply within 45 days. You can appeal a decision by replying to it; Washington residents may then contact the Washington Attorney General.
